Our placement form asks for your name, email address, facility or business name, town, and area of interest. You can also provide a location type and additional details. We use this information to review your location, respond to your inquiry, and manage related follow-up.
Please do not submit patient or resident information, medical details, payment-card information, passwords, or other sensitive personal information through this form.
Employee-program interest requests
Our employee-interest form asks for your name, email address, workplace, town, and permission to contact you about the planned program. We use these details to respond, assess interest at workplaces, and manage related follow-up. An interest request does not create a membership, payment account, wallet, or vending allowance. We do not ask for payment information or a phone number through this form.
Where an inquiry goes
Form submissions are saved in our website’s form system. Placement inquiries are also sent to JRV’s business email; employee-program interest requests are reviewed in the website form system. Authorized people and service providers supporting our website, hosting, forms, security, and email may process the information needed to operate those services and handle your inquiry.
